FND Hope and its affiliates, is running this survey to assess the impact of the COVID-19 crisis on those who have been diagnosed with Functional Neurological Disorder (FND).

We are going to use the results to inform our ongoing organisational response to the pandemic.

It is anonymous and should only take a few minutes to complete.
