2024/2025 Quality Improvement Plan Engagement Survey

Oak Valley Health is developing a Quality Improvement Plan (QIP) for the 2024/25 fiscal year. As we put together our plan, we would like to gather input from our teams, patients and families, and community members.

The Ontario government's Excellent Care for All Act, 2010, mandates the QIP, which is an annual, documented set of commitments hospitals make to improve quality through actions and targets to measure improvement. Ontario Health has identified 4 priority issues: Access & Flow, Equity, Safety, and Experience, and have included some optional indicators to measure performance in these 4 priority areas.
