The Shire of Perenjori is committed to supporting a healthy, connected, and thriving community. As a part of this commitment, the Shire is developing a Local Public Health Plan in accordance with Section 45 of the Public Health Act (WA). This plan will guide how we work with our community, partners, and steak holders to improve health and wellbeing outcomes across the Shire.

Your input is essential. This survey is an opportunity for the community members to share their views on local health and wellbeing priorities, identify any gaps or shortfalls in services, highlight risks, and outline the outcome you would like to see for our community. Weather it relates to physical health, mental wellbeing, access to services, community safety, or social connection-your feedback will help shape future decisions.

The information gathered will be used to develop practical, targeted, and achievable actions that respond to the needs of our community. These actions will form part of the Local Public Health Plan and provide a clear pathway to support and promote healthy lifestyles for all residents of the Shire of Perenjori.

We value your time and insights, and we thank you for contributing to the future health and wellbeing of our community.
