  • DO NOT submit a request for a free PLB if the commercial offshore company you work for already provides a PLB for their employees to use.
  • DO NOT submit a request for a free PLB if you and your family can afford to buy one. Do the right thing and order one online as soon as possible from ACR Artex or McMurdo.
  • Sending you a free PLB depends on how many donations we have received from others. No donations, no PLBs.
  • If accepted to our waitlist, expect to wait over a year to receive a free PLB.
  • YOUR REQUEST WILL BE DENIED if one of these specific sets of official U.S. offshore worker identifications below is not provided in a single photograph:
    • BOSIET/FOET OPITO-approved certification card issued by a U.S. training facility (a paper certificate instead of a card MUST include the cert number, an expiration date, and your U.S. State issued drivers license or U.S. Passport in the same photo as well. Your cert number and name will be verified in the OPITO database.)
    • HUET/THUET OPITO-approved certification card issued by a U.S. training facility (a paper certificate instead of a card MUST include the cert number, an expiration date, and your U.S. State issued drivers license or U.S. Passport in the same photo as well. Your cert number and name will be verified in the OPITO database.)
    • MMC BOOKLET issued by the U.S. Coast Guard. We will verify validity with the USCG.
    • Commercial Fishing License issued by the U.S. (or a U.S. State) Fish and Wildlife Service (your U.S. State issued drivers license or U.S. Passport must be included in the same photo)
    • Crew member license issued by a U.S. State (your U.S. State issued drivers license or U.S. Passport must be included in the same photo, if the license doesn't not include your photo)
    • 1099 from no later than last or current calendar year issued by a verifiable commercial fishing business as proof of Commercial Fishing Deckhand employment (your U.S. State issued drivers license or U.S. Passport must be included in the same photo)
  • All data is required to get on the waitlist.
  • Only one request per offshore worker.
  • Requests are processed on a first come, first serve basis.

    • If your card is two sided, take a photo of both the front and the back, then use an app or screenshot to combine them into one image, side by side, to upload here, so we can see your name, face, and certifications listed in one photo.
    • If your cert card does not have an image of your face, you MUST also include a U.S. drivers license or passport in the same photo.

  • Commercial Fishing License with U.S. State Seal or Crewmember License issued by U.S. State and U.S. drivers license or U.S. Passport all visible in photo:
    • MUST include your valid U.S. drivers license or passport in the same photo with your name and face visible.
  • Current year or last year 1099 from a commercial fishing business (SSN can be blacked out.)
    • MUST include your U.S. drivers license in the photo with the 1099 for us to validate and approve.
