Thank you for your interest in this research study,

You are being asked to participate in a research study conducted by Dylan Serpas, a research assistant with California State University, Fullerton. This study is supervised by the faculty sponsor, Dr. Barbara Cherry, a Professor Emeritus at California State University, Fullerton. If you agree to join this study, you can change your mind later and leave the study at any time.  There will be no penalty or loss of services or benefits if you decide to not take part in the study.  

This study was approved by the California State University, Fullerton Institutional Review Board (IRB). Protocol #HSR-20-21-244.

WHAT IS THIS STUDY ABOUT?

This research study is being conducted to better understand the social experiences of persons with disabilities. We are interested in how aspects of one's identity are related to social experiences and well-being. 

Taking part in this study will consist of completing a brief survey where you complete survey questions about your experiences. The survey is expected to take about 15 to 20 minutes. In order to participate in this study, you must:
1) Be age 18 or older
2) Permanently reside in the United States 
3) Be living with a disability as defined by the Americans with Disability Act

WHAT ARE YOU EXPECTED TO DO IF YOU PARTICIPATE?

If you take part in the study, you will be asked questions about your experience living with a disability in the form of survey questions. Questions will ask about disability-related stress and wellness. You will be able to change your answers on survey items until you complete the survey. 

POTENTIAL BENEFITS

Potential benefits of participating include:
1) Increasing your knowledge about your identity and how it is related to your social experiences
2) Increase the scientific knowledge of of people living with disabilities.
3) At the end of the survey, you will have the option to provide your email address to be entered into a raffle for 1 of 10 Amazon gift cards. Gift card winners will be drawn at the conclusion of the study. You must reach the end of the survey to obtain the option to enter the gift card raffle. 

POTENTIAL RISKS

Participants may feel minimal discomfort, mental distress, and cognitive fatigue answering the survey questions. You may be asked sensitive questions. 

CONFIDENTIALITY AND ANONYMITY

Any information obtained in connection with this study that can be identified with you will remain confidential and will be disclosed only with your permission or as required by law. Confidentiality will be maintained by means of protecting the participant's responses by keeping them on a password-protected computer in the researcher's possession.

The results of this study may be published or presented at professional meetings, but the identities of all research participants will remain confidential.

RIGHTS OF RESEARCH PARTICIPANTS

You can choose whether to be in this study or not. If you volunteer to be in this study, you may withdraw at any time without consequences of any kind.
