Hospital Discharge Experience Survey

Healthwatch Sunderland is your local health and social care champion. Working city-wide, we ensure NHS and social care leaders and decision makers hear your voice and use your feedback to improve services.

We are currently working with South Tyneside and Sunderland NHS Foundation Trust to gather experiences related to hospital discharge. We would like to hear directly from patients, or if that’s not possible, from family members or carers of those discharged from Sunderland Royal Hospital since January 2026.

The survey should only take 5-10 minutes to complete. The closing date for the survey is 13th September 2026.
