2015 Annual Reach Out and Read MEDICAL CONSULTANT Survey |
| 25% |
PLEASE READ:
Each year Reach Out and Read asks the medical leader at every program in our network for important feedback, which in turn helps us understand what is/isn't working at Reach Out and Read programs across the country, what resources you might desire, and also gives us data we need to apply for a variety of grants.
This year we've removed a few questions to make room for questions about your patients' families' library and museum use. With this input, we hope to create an online "tool kit" containing resources and best practices to assist those Reach Out and Read programs interested in educating families about the availability and array of resources in their area. Exposure to more books and different cultural activities broadens children's experiences and, we hope, boosts outcomes for the children you serve.
PLEASE NOTE: Questions refer to the previous 12 months. Please answer candidly. In most cases we ask for your own experience, but there are some questions that target the program as a whole. (And, all questions about providers refer to those providers who see children under age six.)
Thank you very much for your commitment to Reach Out and Read and for completing this 5-minute survey of 20 questions.
