On behalf of Twist Out Cancer and Rolfe Pancreatic Cancer Foundation, we want to thank you for your participation in the program! As we prepare for the culmination of our program, it is critical that we collect key information from all participants. Inspirations, please submit a high resolution (no smaller than 3mb) headshot and an optional 30 second video testimonial that summarizes how you have been touched by cancer and what your experience was like working with your artist. You should submit both the labeled image and video testimonial through dropbox. Please note that your submission will not be considered complete until the survey is complete and the digital assets are uploaded.
