* 6. Describe the problem. Discuss why the project was initiated. Describe the precipitating outcomes and the urgency (examples of information to include but are not limited to: was this a quality improvement project, did it come from a unit based council, was this a cost reduction initiative, was this staff education focused, was it a patient experience improvement project, was it related to patients' family concerns?)
