Question Title

* 1. Your contact information. NOTE: If you are not the parent or guardian of a child receiving a call (for instance you're an aunt, grandparent or family friend offering a gift), please also include contact for the child's parent, so we can assure their knowledge and permission to contact the child.

Question Title

* 2. Can we add you to our email list?

Question Title

* 3. Which scientist are you requesting to call your child? A professional actor will be fully in costumed character, ready to chat about how much we love math and science and keep your child motivated at home! The actor will never break character and reveal they're an actor unless the parent is present and asks them to.

Question Title

* 4. What is your child's name and age? If this is intended for an adult, please specify!

Question Title

* 5. Is this call a surprise for the recipient or are they expecting it?

Question Title

* 6. What are they working on or into right now in terms of math and science (eg, multiplication, fractions, photosynthesis, planets, etc). IMPORTANT: In these 10-minute interactions, the scientists are not functioning as tutors. They are ready to chat with your child, ask them questions, and fan the flames of encouragement. It's helpful to know where they're at so the scientist can prepare for an appropriate conversation with maximum academic benefit, but instruction is not the intent of this interaction.

Question Title

* 7. Any specific academic question they're hung up on and could use a little extra encouragement? Again, the scientists are not functioning as teachers or tutors, but are happy to prepare to discuss a particular point of difficulty in a positive way if it's helpful.

Question Title

* 8. OPTIONAL: Any other personalized message you'd like the scientist to include? Some examples might be:

"In my lab we all take turns sharing equipment. Did you know scientists always share their toys and books?"

"It's very important to keep our laboratory clean and that's why we all help with chores."

or even something to reassure them about something hard, such as:

"I happened to get a letter from your Grandma Ruth! She mentioned that she's safe and she really misses you, and can't wait to play games and puzzles with you again."

This can be personalized as much as you want, if you want. We know kids and families are doing their best to adapt to strange and changing circumstances, so we're happy to provide little positive nudges to help adjust to the social distancing dynamic.

Question Title

* 9. Please select three options for scheduling. Please schedule at least 24 hours in advance, as the scientists may be on another call and not always available for same-day appointments. The scientists are currently available the following times, or by special appointment with advance notice.

Question Title

* 10. We value this personalized experience around $25, but will gratefully accept payment on a sliding scale starting at $10 up to whatever you feel the experience is worth. Your payment is supporting working artists during a time of grave uncertainty, and for that we are deeply appreciative! Please go to https://us.patronbase.com/_Matheatre/Products/Donate to make your payment and customize your amount. Upon receipt of payment and this complete questionnaire, the scientists' manager, Sadie Bowman, will contact you with a Zoom link for your child's experience. Please be sure to log into Zoom at the appointed time. Cancellations must be received by email no less than 24 hours in advance or the appointment is forfeit. If you need to cancel we will do our best to reschedule, but regrettably cannot offer refunds.

Matheatre's professional actor-educators have experience working in schools with children from preschool to high school. They have years of experience playing the roles of Marie Curie and Isaac Newton (as well as other STEM-centric historical figures) in professional stage shows for schools and museums worldwide, and will fully commit to the characters during your call. We will never share your child's information, record a call, or attempt to contact your child without the express invitation of a parent. A parent is welcome to be present for the duration of the call. Our actors are imaginative conversationalists but will not tolerate abuse or harassment of any kind. Impropriety on the part of a call's recipient will result in termination of the call at the discretion of the actor, with no refund.

Each video call will last approximately 15 minutes, and Zoom is our default (and preferred) video platform. If you prefer to use Google Hangouts, FaceTime or Skype, please specify.
