COVID-19 Incident Feedback Form Phase 2 (September 2020 - February 2021)

The Southeast MN Disaster Health Coalition continued response actions for COVID-19. These actions included, but were not limited to, continued filling PPE supply needs, addressing staffing issues in long term care and congregate care facilities, and working with partners to tackle vaccine allocation issues. As we reach one year of response to COVID-19 it would seem to be a good time to pause and reflect on what has gone well and what opportunities for improvement we discovered. Coalition leadership would greatly appreciate you taking a few minutes to complete this survey. This information will be used in the creation of an after action report that will be shared in the future.
