Dear Committee Chairs and Staff,

In an effort to develop a means to assess your committee's initiatives or events BEFORE they take place, the following assessment form has been created. Please utilize this form to share how your committee is progressing forward with preparation for your upcoming initiative or event identified in the CBI 2017-2020 Strategic Plan. 

Also note that if you have more than one initiative or event to assess, kindly complete a separate survey for each initiative or event.  Please include all relevant information, sharing specifics about preparation as well as any concerns you anticipate.  Including specific information in the comment areas for each question will be particularly helpful.

After your initiative or event has been completed, please fill out the separate AFTER Information Feedback Form.

The information gathered in all survey monkey assessments will play an instrumental role in determining the implementation of the CBI Strategic Plan as it relates to annual operations and the future direction of Congregations Beth Israel.
