By completing the questions below you are registering your interest to be a witness in support of our unions claim for Travel Time for Disability employees.

We are looking for members that are paid under the Social, Community, Home Care and Disability Services Industry Award 2010, known as the SCHADS Award as well as other criteria so it is important that you complete the questions below.

Our Union is required to have evidence filed by 28 June 2019. If our Industrial team think the information that you provide below would make you a good candidate to be a witness they will be in contact shortly. So it is important that you provide your contact details below.
