Preventive Care Verification - Welcome

Routine preventive exams and screenings can provide you valuable information about your health. Early detection of potential health problems can help you better treat and manage them. 

To meet the Liberty Healthcare preventive requirements, you must have an annual physical and complete a minimum of 3 of the health screenings listed below in accordance to medical guidelines between 2/1/23 and 3/15/25.

Annual Physical REQUIRED*:
 - One annual physical per plan year

Health Screens - Choose any 3:
 - Mammogram (women ages 40 to 69): Every 12 - 24 months
 - Cervical Cancer Screening (women ages 21 to 64): Every 36 months
 - Colorectal Cancer Screening (adults ages 50 to 75): Fecal Occult Blood test every year, Cologuard every 3-years, Sigmoidoscopy every 5-years, or Colonoscopy every 10-years.

- Vaccinations (Flu, COVID, Pneumonia)
- Nutrional Visits
- Diabetes Screening
- Cholesterol Screening
- HPV Testing

* Your medical insurance covers one physical per plan year with no co-pay (4/1 - 3/31). 
Some guidelines may vary based on medical history. Please consult your physician for additional guidance.

Note: This form should only be completed for screenings that have not yet or will not come through via BCBSNC or Aetna claims. For example: Recently hired employees, employees/spouses who are new to the BCBS medical plan, or if you believe a screening may have been coded as diagnostic rather than preventive.

Important: You must complete the entire survey to obtain credit. Spouses must complete a separate survey to receive credit.
