At-Risk Afterschool Meals Survey

The Maryland Out of School Time Network (MOST) is committed to increasing the ultilization of the federal At-Risk Afterschool Meals Program throughout the state. The At-Risk Afterschool Meals Program provides reimbursement to eligible afterschool programs that provide meals and snacks that meet nutrition guidelines. Programs must meet at a location that is either at or near a school where 50% or more of students are eligible for Free or Reduced Meals.

The following survey is designed for afterschool program providers and is best completed by the site director. The survey should take about five minutes to complete.
