CONSENT FORM

You are invited to take part in a research study about nurses’ interactions with patients diagnosed with borderline personality disorder. The researcher is inviting all nurses that have interacted with patients diagnosed with borderline personality disorder within their job duties in outpatient, in patient, emergency room, psychiatric unit, etc. to be in the study. This form is part of a process called “informed consent” to allow you to understand this study before deciding whether to take part.

This study is being conducted by a researcher named Nicole York, who is a doctoral student at Walden University.

Background Information:

The purpose of this study is to identify factors that influence nurse’s social distancing from the patient diagnosed with borderline personality disorder.

Procedures:

If you agree to be in this study, you will be asked to:

·        You will be invited to participated in a one-time 10-minute no more than 15 minute on line survey questionnaire

·        This is completely voluntary and information collected will be used for research purposes

·        Here are some Sample Questions that will be asked:

o   What is the highest nursing degree to you have?

o   What settings have you been in contact with an individual with Borderline personality Disorder in your professional experience?  

Voluntary Nature of the Study:

This study is voluntary. You are free to accept or turn down the invitation. If you decide to be in the study now, you can still change your mind later. You may stop at any time.

Risks and Benefits of Being in the Study:

Being in this type of study involves some risk of the minor discomforts that can be encountered in daily life, such as fatigue, stress or becoming upset. Being in this study would not pose risk to your safety or wellbeing.

Your participation in this study will benefit the larger community by providing information to organizations to assist with trainings and support for yourself and others within your profession.

Payment:

There is no form of payment for your participation in this study.

Privacy:

Reports coming out of this study will not be shared. The researcher will not know who you are. The researcher will not use your personal information for any purpose outside of this research project. Data will be kept secure by Nicole York in a protected password protected file.  Data will be kept for a period of at least 5 years, as required by the university.
