This is your personal (and confidential) 'honesty' audit. There will be five focus areas - each one asking various responses from ratings to verbatim comments.
Some will be easy for you to respond to, others may require you to reflect deeply, and some may possibly make you feel uncomfortable.
You may want to do the assessment in more than one sitting, you can press save and return to the document at any time.
As an 'honesty' audit, sometimes the first thought is the one you know has to be recorded, at other times, you reflect for a moment, and the 'hard-truth' comes. You know with honesty, that this is the response you need to record.
The goal is to create a base-line for us to work with (in conjunction with the program that I have designed for you).
Please complete the document within (one week) of receiving it. I will receive an alert once it has been received. At this time, I will review it and arrange to forward it to you with my compliments.
