The UG-WYCOKCK Health Department and KU are partnering to conduct a survey to identify top health issues in the county.

Please fill out this survey only once and if you live in Wyandotte County.  This survey is research aimed at finding out about top health issues for Wyandotte County residents. This information is provided so you can decide whether you want to take the survey. It should take you 15-20 minutes to finish.  Taking this survey is voluntary (meaning you do not have to take it and you can stop at any time). There are no risks to taking the survey. By taking the survey you are helping to select top health issues, which is the only benefit. Even if you start the survey, you can stop at any time. Your responses are confidential; only total survey results will be shared.  Your name or other type of personal information will not be collected as part of this survey.
