Woodland House Feedback

Dear Families,

As you may be aware we have recently opened Woodland House, a centre for families who have had a pregnancy loss, or a baby who has died after birth. You have visited the house for an appointment recently, as part of the care following your families bereavement. You will have also had part of your follow up care in the Bereavement Teams old spaces within the main hospital building (for example the room we used to use on the 1st floor for taking blood tests, or the family visiting room on the lower ground floor where you may have come back to see your baby). Because you have had outpatient appointments with the bereavement Team within both settings it would be really helpful to us if you would give us your feedback on Woodland House.

Thank you for taking the time to complete this survey. We are so sorry for the loss of your baby, and that you have had to visit us at all.
