2021 WSEMA Annual Conference Call for Presentations and Participation Opportunities

Please complete the following survey as completely as you are able. Responses may be submitted through Friday, August 13. Due to time constraints the Conference Committee will be reviewing submittals on a rolling basis and reserves the right to make selections prior to the submittal deadline so presenters are encouraged to submit early! 
1.CONTACT INFORMATION. Please enter the best contact information to reach you regarding your response form.
2.OPPORTUNITIES TO PARTICIPATE. Please select the opportunity that you are submitting a proposal for or expression of interest in.
3.FOR PRESENTATIONS/PANEL DISCUSSIONS. If you are proposing a presentation or panel discussion please provide a title and brief summary of the proposed content in 500 words or less.
4.FOR PROJECT WORKSHOPS. If you are proposing a project workshop, please provide a summary of the project or effort you would like to present and the challenge or opportunity you hope to explore with the group in 500 words or less.
5.FOR MEMBER FACILIATORS. Please provide a brief explanation of why you would like to volunteer and your experience in facilitating group discussions.
6.FOR INFORMATIONAL POSTERS. Please indicate the topic of your poster and the information you hope to convey to WSEMA membership.
7.CONFERENCE ATTENDANCE. Do you plan on registering for and attending the full conference?
8.APPROVAL TO ATTEND. Do you have approval from your organization to attend and participate in the conference?
9.ABILITY TO ATTEND. Are you able to commit to holding the dates of September 21-22 through August 27 (when the program is finalized) and participate in the conference on the day assigned for your presentation or workshop?
10.MATERIALS. Please describe any materials you will be using for your presentation or workshop.
11.AUDIO/VISUAL/IT SUPPORT. Please describe any audio/visual or information technology requirements for your presentation or workshop.
12.ACCESSIBILITY. Please describe any accommodations that may be needed to support you in participating in the conference.
13.QUESTIONS/ADDITIONAL COMMENTS. Please provide any additional comments or provide any question you have here.
