Dear Participant,

We invite you to participate in the Whole Person Care Model Study. You are invited to participate in this study to help us further understand whole person care. Participation in the study is voluntary and no monetary benefits are associated with the study. We anticipate at least 300 participants throughout the study.

Description of the Study
The purpose of this study is to validate the Loma Linda University Whole Person Care Questionnaire (LLU WPC), a 40-item questionnaire that is associated with the Loma Linda Whole Person Care Model (CLEAR Model). The study requires you to complete a 111-item questionnaire, Whole Person Care Questionnaire (WPC Questionnaire), which includes questions that relate to personal wellness, stress, empathy, listening, spiritual life, wholeness and whole person care (the 40-item LLU WPC is contained within the 111-item WPC Questionnaire).

Students and employees from Loma Linda University have been immensely helpful in helping develop the Whole Person Care Model, so we appreciate your continued help in this research. Please take the time to answer each question based on your experience and/or philosophy as an individual. The questionnaire can be completed in about 20-23 minutes, but there is no time limit.

Your responses are confidential and we will keep your records private to the extent allowed by law. Every participant will be given a unique Research ID. In order to reduce any bias that may exist, the investigators/researchers will only be given the Research ID and will not be able to link responses to individuals.

Risks
There are minimal risks to those who choose to participate in the study. There is the possibility you may experience some psychological stress when responding to the 111-item questionnaire that includes questions regarding personal health, spiritual life, burnout, empathy and listening.

Benefits
You may receive benefits from reflecting on your life through the WPC Questionnaire. You may also contribute information that will enable us to better understand whole person care. Finally, you may contribute information that will help others learn about whole person care.

Voluntary Participation
Participation in this study is completely voluntary. If you decide not to participate there will not be any negative consequences. Please be aware that if you decide to participate, you may stop participating at any time.
