Women's Views on Perinatal Care Survey

Introduction

Reach is conducting a national survey that looks at the needs and interests of pregnant women. The survey focuses on your thoughts about the use of digital technology, the quality of care you are receiving, and supportive health information that you may need at this time.  This survey is intended for expectant and new mothers who delivered within the last 12 months. It is also intended for women who plan to be pregnant in the next twelve months.

No personal information provided will be shared with any other parties. My personal / identifiable information will be kept completely confidential. This is an anonymous collection of information and be part of an aggregate analysis.  Any exceptions to this will be at my discretion. I agree to the processing of my data in accordance with the information provided herein.
