Disaster Preparedness and Response Survey

St. Anthony launched the IOCC Homefront Ministry in late 2019. The ministry purpose is to help prepare our parish and parishioners to respond quickly and efficiently to disasters and/or local emergencies.

This short survey is designed to gather information regarding the skills, experience, and tools of St. Anthony parishioners so we can better understand our congregation and respond appropriately when disaster strikes.

We ask that each adult submit an individual survey. Your answers will not obligate you in any way, however, we may follow up to see if if you would be willing to help if related circumstances arise.
