Good Sportsmanship and Phoebus Awards

Use this survey to nominate your choices to receive these two club awards. These awards are based on nominations from the membership; the persons receiving the most votes from members will be awarded a framed certificate at the Awards Luncheon. (If ties occur, they will broken by vote of the Board.)

NOTE:  After October 30, you will receive a second form to report any titles earned by your dog since October 31, 2017. All dogs who earn an AKC title will be honored at the upcoming Annual Awards Luncheon.
