Wisconsin Head Start State Supplement Grant Application

Before you begin the survey, please make sure you have the following four documents ready to upload:

Federal Program Schedule
DCF Contracts Form 5403
Most Recent Federal Notice of Award
Most Recent Community Needs Assessment

You will be asked to identify what MOUs or partnerships you have in place with school districts and other community partners. Be prepared to answer a few questions about these.

Finally, you will also be asked to identify and provide contact information for who will be the signatory for the contract and who needs to be CC’d on the contract signing. Make sure you have these ready to fill in.
