Wisconsin Community Match Survey

Thank you for participating in the Wisconsin Community Match.  Community Match is an online resource that helps you build partnerships in your community. With Community Match you are able to access your community partner's contact information, shared interest in collaborative activities, and resources to build and strengthen partnerships. Hundreds of leaders from hospitals, WIC agencies, home visiting, health departments, community benefits programs, prenatal/outpatient clinics, breastfeeding coalitions, and community groups have participated. 

We just need some basic information in order to match you with partners in your community. Let's get started!
