CAPTE requires that accredited PTA Programs acquire formal assessment information.  Also, we would like to know how we're doing!  

This survey is composed of four sections as follows : 

      * Assessment of the Program Based on Student Performance
               -provide feedback about the Program because of what you observed student(s) do at your facility
      *Assessment of the Program Based on Interaction with ACCE
               -provide feedback about the Program because of the interactions you experienced with the ACCE
      *Assessment of ACCE
               -provide feedback about the ACCE
      *Summative Comments
               -provide overall impression of the Program and the ACCE
