The questionnaire consists of a number of statements. Rate yourself against each statement as honestly as you can using a scale of 1-5 where 1 means that the statement is completely untrue, 5 means it is completely true.

The questionnaire is about you as a ‘whole person’, inside and outside work. However if it is difficult for you to answer on that basis, then focus on how you are at work.

You must answer each question. if you have difficulty answering any of the questions please tell us about this in the comment box at the end of the questionnaire.
