Nominate PNP of the year

The PNP of the year award is presented by the AZ NAPNAP to a pediatric nurse practitioner who exceed expectations in performing their duties. This nursing professional is recognized for his or her admirable character, selfless dedication, and outstanding effort. Perpetually raising the bar for all, they help establish a higher standard quality of care.

Public recognition at the 4th Annual Pediatric Symposium and Welcome Reception
Award
Reward sponsored by AZ NAPNAP
Qualification criteria for the nominees may include but are not limited to:

Current NAPNAP member
Service as NAPNAP chapter officer, committee chair, or task force member
Activities related to state or regional health policy initiatives
Presentations to state and local organizations that address pediatric healthcare concerns
Original publications that may have affected the delivery and practice of pediatric healthcare services

Application Requirements:

A double-spaced, typed narrative description (500 words or less) of the reasons this nominee should receive the award according to the criteria listed above
1.Write the name and email of the person you would like to nominate for PNP of the year. This award will be presented Friday November 1st in conjunction with the 4th Annual Pediatric Symposium Welcome Reception. Selection will not be based on the ability to attend the Welcome Reception.
2.Describe why you are nominating this individual for their accomplishments to NAPNAP and our community within the past year.
3.Will you be attending the Welcome Reception and would like to help present your nomination should they be selected as PNP of the year?
4.Will the nominee be attending the conference or Welcome reception to the best of your knowledge?
