Seneca Nation Disabilities Services Survey 2026

SENECA NATION YOUTH DISABILITY CARE SURVEY 2026

Seneca Nation Disability Services' mission is to focus on our members under 60 years of age with disabilities. Our goal is to make our territories disability friendly and ensure members receive a quality life.
With goals to improve the general welfare of Seneca Nation members with disabilities and reduce the associated cost of health care services by providing supportive services, we hope to inform Seneca Nation members of the supports and services that are available.
By sharing your lived experience, through your participation in this questionnaire, you will assist and guide Seneca Nation departments in better understanding barriers Seneca children and their families face when seeking disability services and care. One survey can be completed per family structure. If there are more than three children with disabilities within a family structure, please complete two or more surveys.
Nya:wëh for your time.
