Join the Party Planning: Help Us Select the Top 10 “Primary Care Promoters” of the Decade!

To celebrate the PCPCC’s 10-year anniversary, we would like to feature your favorite “primary care promoters,” who will be invited to inspire us with short “TED talks” at our Annual Fall Conference, November 10th. Please tell us who, in your opinion, has done the most to promote primary care since the PCPCC was founded 10 years ago. We're looking for nominations that reflect the diverse stakeholders that are part of the PCPCC and who’ve been key in driving primary care transformation in your community (PCPCC staff and Board members excluded).

Once we receive your nominations, we will use social media to crowd source who will be selected to present the TED talks – so stay tuned.  If your nominee is a selected speaker, he/she will receive free registration to the PCPCC’s Annual Fall Conference (as well as much-deserved public recognition) and YOU will be invited to introduce them!

For the various categories below, please reflect on the past 10 years and nominate outstanding individuals who have been strong "primary care promoters." Please feel free to nominate in as many categories as you'd like:
