Annual PRAPARE® Interest and Experience Survey

The Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences (PRAPARE®) is a national effort to help community-minded organizations assess social risk factors and use that information to accelerate care improvements at the patient/client, provider, and system levels.  

This survey, led by the National Association of Community Health Centers (NACHC) and the Association of Asian Pacific Community Health Organizations (AAPCHO), is designed to: 
  • Assess current PRAPARE® users’ experiences
  • Share promising practices
  • Document challenges and implementation support needs among current PRAPARE® users and those that have not yet implemented PRAPARE®.  
The feedback you provide will be used to develop or refine PRAPARE® resources, training, assistance in order to best support the community of PRAPARE® users. 
 
Participating in this survey should take you between 10-15 minutes to complete.  
 
Who Should Complete this Survey?  
Each organization currently using, testing, or considering the use of PRAPARE®  should complete this survey.

This includes health care  providers or delivery systems, social service providers, health plans/payers, and other direct services organizations whose staff use PRAPARE® electronically or in paper form to screen for and record individual patients’ social risks. In order to avoid multiple responses from the same organization, those organizations involved in the direct deployment of PRAPARE® should assign one staff "champion” who can speak to your organization’s experiences using PRAPARE® and remaining needs. 

Those organizations or individuals NOT involved in the direct or “on the ground” use of PRAPARE® to screen patients/clients  we ask that these organizations (PCA, HCCNs, membership organizations) forward this survey on to health centers and other direct service organizations that are currently using or interested in using PRAPARE® . If you are a Researcher,  do NOT complete this survey. You may email prapare@nachc.org regarding your PRAPARE® -related research interests and activities.

Confidentiality of Responses
Only NACHC and AAPCHO and its affiliates will have access to your responses, unless your permission is provided otherwise. Results from this survey will be disseminated in such a way that you or your organization cannot be individually identified.
