The Leadership Task Force for Dementia Friendly Champaign-Urbana is pursuing designation as a Dementia Friendly America (DFA) community. DFA is a national network of communities, organizations and individuals seeking to ensure that communities across the U.S. are equipped to support people living with dementia and their care partners.

Dementia is a general term for a group of conditions that cause a decline in thinking, memory, and reasoning abilities, significantly affecting a person's life. The most common form of dementia is Alzheimer's Disease, although there are over 100 different types of dementia including Vascular dementia, Lewy body dementia, and Frontotemporal dementia.

We need your input as part of the Champaign-Urbana community to help the Leadership Task Force develop and prioritize an action plan to address gaps and opportunities for persons with dementia and their caregivers. The following is a short survey and responses are anonymous. Your participation is greatly appreciated. Thank you!
