Survey for J-1 Trainee/Intern

We hope you are having both a professionally and culturally enriching experience in the United States. To comply with J-1 visa regulations, you are required to complete a Midpoint Evaluation if the duration of your program is longer than six months. The survey should not take more than 10 minutes and should be submitted within 10-15 calendar days of your program midpoint. Please make sure to answer the questions completely.
