LIFT Student Application: Summer 2026

Leading Inclusive Futures Together (LIFT)

The LIFT Summer Mentorship Program runs for two different weeks in the summer: July 20-24 and August 10-14, 2026 at St. Joseph's Health Centre. Participants will be selected to attend one of these weeks only.

The Health Centre is located at 30 The Queensway in southwest Toronto. The closest TTC subway station is Dundas West.

In LIFT, high school students of Indigenous, Black and various racialized backgrounds are given the opportunity to experience a healthcare environment.

As an alternative to a written submission, you may submit a maximum 6 minute video to LIFT@unityhealth.to in the following formats: MP4, or MOV. If you plan to do this, please write "video" in the section below and send in your video not more than 5 days after you submit this form.

Here are some examples of things you could include in your written or video answer:
  • Skills and knowledge you have
  • Life experiences you have had
  • Description of your personal, academic and/or career goals
  • Challenges you have overcome and/or barriers that you face
  • Opportunities that this program would provide you with
  • Anything else you think is relevant.
Please note: If you opt for a written answer it should be at least 100 words long. This is a minimum requirement of about 4-5 sentences. There is no maximum limit. The longest answers past applicants gave us were around 550 words long.(Required.)
