PSE Students with Disabilities Questionnaire Consent Form

Thank-you for accessing the PSE Students with Disabilities Questionnaire through Survey Monkey.  The purpose of this study is to assess post-secondary education (PSE) students with disabilities’ uses and needs of online communities (e.g., Facebook, Twitter) and electronic mentoring (e-mentoring) opportunities. The study is being conducted by investigators from George Brown College and the University of Ontario Institute of Technology with the assistance of the National Educational Association of Disabled Students (NEADS).

Your feedback will be collected through an online questionnaire which will ask you a series of questions and should take about 5-10 minutes to complete. Participant responses will be combined and analyzed together. The findings of the study will be shared through presentations and publications. The findings will also inform the development of an online network for PSE students with disabilities. This resource will create opportunities for learning and social support through online-community building and e-mentoring. 

This online questionnaire is completely anonymous. You are not required to provide any personal information such as your name, address or telephone number, and you don’t have to answer any questions you don’t want to.  The survey system will not record your e-mail address or IP (Internet protocol) address. The use of Survey Monkey to collect data involves uploading information to an American server. As with all data uploaded to Survey Monkey, according to the USA Patriot Act, it has the potential to be accessible to American regulatory bodies should it be deemed to potentially contain relevant information.

Your participation is voluntary.  If you do not wish to participate, simply do not start the questionnaire. If you start the questionnaire and decide you no longer want to participate, do not submit your responses and close the browser tab.

If you agree to participate in the study by completing the questionnaire, please note that you must complete it in one sitting (in other words, the system won’t let you save your questionnaire responses and return to complete them later).  Please also note that you will not be able to withdraw your responses once they have been submitted as we cannot link questionnaire responses back to you.
