What strategies should HIPMC consider to achieve CHIP goals?

Based on input from HIPMC members, we've set out an overall goal for each priority in the CHIP (community health improvement plan). We then created the driver diagrams below based on members' input about a few main factors that influence that goal (primary drivers) and several factors that influence the primary drivers (secondary drivers). 

Please review the 3 driver diagrams shown below and let us know where your organization is working. You can select as many or as few drivers as are applicable.

Combined with feedback from other HIPMC partners, this will tell us where HIPMC has the most momentum and where gaps may lie. Then, at a later stage, we'll work to add programs and activities that will enact the drivers.
