Parent Needs Assessment

The School Counseling department would like to survey parents, teachers, and students in order to develop and provide programs that meet the students' needs.  This survey will help us determine the needs of students individually as well as which topics we need to address in our School Counseling programming.  This survey is anonymous, as we require no identifying information other than the student(s) grade level(s).  We would appreciate your time answering the below 10 questions.  Thank you for helping us better meet the needs of your child(ren). - MHS School Counseling Department (J.
