I have been asked to participate in a research study that investigates the opinions on modes of transportation in Northeast Ohio.
In participating in this study, I agree to have my data used for research purposes. This includes any disclosure of location given. Data will be limited to opinions, transportation/income demographics, age range, and the metropolitan area you state you are located in. Incomplete surveys will be discarded.
I understand that:
i. There are personal questions being asked in this survey, but no names or identifying information will be collected.
ii. There are no expected risks in completing this survey.
iii. There are no direct benefits associated with completing this survey.

v. I may refuse to participate or may withdraw from this study at any time without any negative consequences by simply exiting the survey by closing the browser window. Also, the investigator may stop the study at any time.
vi. I also understand that no information which identifies me will be released without my separate consent, and that all identifiable information will be protected to the limits allowed by law. If the study design or the use of the data is to be changed, I will be so informed and my consent re-obtained.
