Please note that current employees (including expatriates) who have previously provided proof of vaccination can use this tracker to provide your booster information (which is required for international travel)--but you do not need to re-submit proof of your initial full vaccination; instead you can skip those questions.  

Any information you provide in this form is secure and will be collected, processed, and stored confidentially in accordance with applicable law as well as Newmont's General Privacy Statement and Human Resources Related Privacy Statement, to the extent applicable. 
Your proof of vaccination will be shared only on a need-to-know basis in accordance with applicable law for legitimate health and safety purposes (i.e., to comply with Newmont's vaccination requirements in the U.S.).
Newmont will only publish location-wide statistics of the number of employees and third party workers who have been vaccinated, but NOT any information related to you specifically.  If you have ANY questions, please contact your leader, Human Resources or Health, Security & Safety.
