Dear Walk10Blocks app study participant,

We are contacting you because you consented to participate in our walking app feasibility study. We are inviting you to complete a short survey on your experiences with the Walk10Blocks app study. The survey will take less than five minutes to complete.

Voluntary Participation
Your participation in this survey is entirely voluntary. Please note that you consent to participate by proceeding with and completing this survey. You  may request the withdrawal of your data by contacting walk10blocks@arthritisresearch.ca. An exception would be when your data has already been anonymized and combined with other data.

We are using SurveyMonkey to collect this data. The data is sorted in the United States using the US Swiss Save Harbour framework. Confidentiality 
All information collected during this survey will only be accessed by the Walk10Blocks research team. The data will be used for improving the Walk10Blocks app. As per guidelines of the University of British Columbia, the data in this survey will be kept for 5 years to be used for future research. The data will be backed-up in a password-protected document, in Arthritis Research Canada (Richmond, BC) using a 128-bit SSL encrypted server.
