Volunteer Award Nomination Survey

ESS Support Services is proud to recognize volunteers who make a meaningful impact within our programs and community.
We invite staff, volunteers, clients, and families to submit nominations for the following awards:

Outstanding Volunteer Service Award - To recognize a volunteer who has demonstrated exceptional service, impact, and commitment within the past calendar year.
Eligibility Criteria:
Minimum 1 year of service
Minimum 50 volunteer hours in previous calendar year
Volunteer in good standing
Demonstrated excellence through reliability, initiative, engagement, and positive impact

Rising Star Volunteer Award - To recognize a newer volunteer who demonstrates exceptional promise, initiative, and positive impact early in their service.
Eligibility Criteria:
Less than 12 months of service
Minimum 25 volunteer hours
Volunteer in good standing
Demonstrated exceptional impact on clients, programs, staff and other volunteers during their initial period of service.

All nominations will be reviewed by the Volunteer Advisory Committee (VAC) in accordance with the Volunteer Recognition & Awards Procedure.
