Caregivers are the unsung heroes in the American healthcare system. You support family, friends, and neighbors navigating care. As you take this survey, focus on your experience caring for one person ("Care-Recipient") and one significant issue they have faced or are currently facing. If you have had experiences with more than one person in your care (for example, a child with a chronic illness as compared to an aging parent), please complete individual surveys for each Care-Recipient.

At the end of the survey, you will be asked:

* what is your biggest challenge navigating healthcare; and
* what is one change that would make the greatest difference to you.

Keep these two questions in mind as you got through the survey--your input will help us change the system. This survey will take approximately 10–15 minutes. Thank you for your time.
INFORMED CONSENT

Participation in this survey is voluntary. Your responses will be kept confidential and used solely for research purposes to better understand and improve support for caregivers. No personally identifying information will be collected unless you voluntarily choose to provide it.

Please note: Being a caregiver is hard. Navigating healthcare is hard. Completing this survey may result in emotional discomfort; please be considerate with yourself, skip any question, and withdraw from the survey at any time.
