Healthy Chicago 2.0 Seed Grant Intent to Apply

Thank you for your interest in the 2018 Healthy Chicago 2.0 Seed Grant funding opportunity. Please complete and submit the survey below to inform us of your intent to apply no later than Thursday, March 8, 2018. 

Interested applicants are strongly encouraged to complete this survey to ensure eligibility prior to submitting the full proposal. Please note that submission of this survey is not binding nor is it a prerequisite for submitting an application.
