It has eleven questions and should take less than 5 minutes to finish. At the end of the survey, you will be asked if you would like to participate in a more in-depth, supplemental survey with eleven additional questions. All information and responses you supply will be kept anonymous, and you will not be subscribed for additional emails. After finishing, we ask that you distribute this survey as widely as possible.
