Thank you for participating in our survey! We are continuing to design the One Mind Flourish@Work program to help build greater opportunities for individuals with Serious Mental Illness (SMI), with your input. In the first quarter of 2026, we conducted our first survey to better understand the experiences of individuals with serious mental illness in the workplace, including the challenges they face and the opportunities that help them thrive. Our second survey will build on what we learned to further identify what employers and managers most need to learn, practice, and change in the Flourish@Work curriculum.
Your answers are completely anonymous and key findings shared publicly will not identify individual participants. This survey takes about 10-15 minutes to complete, but you may take longer. Individuals that complete the survey will be entered into a raffle for a $50 dollar gift card for participating.
This survey will benefit people with SMI by helping us build practical tools for employers. There is some risk of emotional disturbance, as questions may prompt reflection on unpleasant experiences. You may skip any question or stop taking this survey at any time. Please answer the survey only once.
