Effective January 1, 2014, a new comprehensive PDQ (Position Description Questionnaire, or job
description form) was rolled out for all UWOSA members. Many of us have since expressed our
frustration with the lack of response after submitting our PDQs to our supervisors.

Acting on these growing concerns, the UWOSA Executive is soliciting your help in determining
the extent of the PDQ evaluation delays. Please complete this very short survey and send us
your (preferably, signed) PDQ. The importance of your participation cannot be overemphasized.
