COVID-19 Request for Audit Postponement and/or Remote Audit using ICT

As required by ASACB Policy, the following questions must be answered in detail and returned to ASACB in an effort to establish an appropriate course of action.  When the survey refers to "client" it means your company:
1.Please provide the name and address of your company.

3. When will the facility/organization be able to function?(Required.)
4.When will the client be able to ship products or perform the service defined within the current scope of certification?(Required.)
5.Will the client need to use alternative manufacturing and/or distribution sites?(Required.)
6.If so, are these currently covered under their current certification or will they need to be evaluated?(Required.)
7.Does existing inventory still meet customer specifications or will the client contact its customers regarding possible concessions?(Required.)
8.If the client is certified to a management system standard that requires a disaster recovery plan or emergency response plan (e.g. ISO14001, etc.), has the client implemented the plan and was it effective?(Required.)
9.Will some of the processes and/or services performed or products shipped be subcontracted to other organizations? If so, how will the other organizations’ activities be controlled by the client?(Required.)
10.To what extent has operation of the AQMS or QMS been affected?(Required.)
11.Has the client conducted an impact assessment?(Required.)
12.Identification of alternative sampling sites, as appropriate.(Required.)
13.Identification of Information and Communication Technology capabilities to assist in the performance of a remote audit?  (i.e. Web-ex, Teleconferencing, Facetime, Google duo, Microsoft+):(Required.)
14.What type of alternate audit method are you requesting? (100% ICT remote or Blended ICT remote and On-site or Postponement of audit)
15.Why are you requesting an alternate audit method? (i.e. Facility is not allowing outside visitors, Employees are working remotely, Shut down due to current situation as a result Covid-19)
