Introduction


North Western Melbourne Primary Health Network (NWMPHN) is reviewing our Community Participation Plan to ensure it continues to support our community participation activities, and the needs of the communities we serve.
 
The current Community Participation Plan outlines our goals for community participation and the steps we are taking to achieve them.
 
We would like to hear your thoughts on the current Plan and invite you to complete this 10 minute survey. The feedback we receive will inform the development of the 2023 – 2026 Community Participation Plan.
 
All responses in the survey will be anonymous (we won’t be able to identify you). It will close at 11.59pm Sunday, 19 February 2023.

Prize draw
 
Once you have completed the survey, you will be invited to enter a draw to win one of four $50 visa gift cards. Your name will not be linked to your survey responses.
 
You will only be able to enter the prize draw once. We will announce the winners (first initials and surnames) on Friday, 24 February 2023 via email, and winners will be contacted directly.
