Restrictions: This survey is intended only for Hmong residents of Wisconsin.
The purpose of this survey is to determine your desires for our future. As the Hmong community, we have been a part of the United States for nearly 50 years, witnessing significant changes. Now, as we look ahead to the next 50 years, your input is crucial in shaping our future.
Your participation in this online survey is voluntary. You can choose to participate or not to participate and may leave the survey at any time. Your decision will not affect the services or support provided to you. Importantly, this survey is designed to be completely anonymous. We are not collecting any information that could identify you.
We will ask you to respond to questions about what you would like to see for yourself, your family, and your community so that we can create the future we want. We will also ask you to provide demographic information such as age, gender, ethnicity, etc.

Duration: This survey should take approximately 25-30 minutes to complete. After completing the survey, you will be able to enter your email for a drawing to win a $500.00 gift card.
